という記事の中で気になる記述が・・・

なるべく家で暮らしたいと思っても、要介護度が重くなるにつれて難しくなる。看護師やヘルパーが夜間の緊急コールに応えるサービスも介護保険にはあるが、他のサービスと併用しにくく、普及が進まない。併用を阻む背景には、要介護度に応じてサービス量を定めた「支給限度額」の問題がある。来年度の報酬改定に向けて、見直しが検討されている。

記事中において、現場のケアマネージャーたちが「支給限度額」と戦っていることを、どちらかと言えば、好意的なニュアンスで書かれているとおもいます。

　支給限度額の引き上げは、介護報酬改定のたびに浮上しては消える課題だ。厚生労働省は一貫して見直しに消極的。見直しは介護保険財政への影響が小さくないからだ。限度額を超えて利用する人は要介護５でも６％にとどまる。

　この問題が冷静に議論されないのは、個々の介護メニューである「ケアプラン」への不信があるからだ。前回報酬改定に際し、厚労省が限度額を超えたケアプランを調べたところ、ヘルパー利用に偏った単調なプランが多く、中でも家事援助の利用が多かった。「ヘルパーを家政婦代わりに使っているのではないか」との批判は根強い。一方で、認知症の日中独居の人が「家事援助」の名目で見守りを利用している可能性も否定できない。

世の中には素晴らしい、ケアマネージャーさんがたくさんいるのに・・・
お年寄りと家族の絆を再生できるようにと、法律以上の仕事をする人がいるのに
ご本人やご家族がサービスに依存し、関係が遠くならないように

いろいろ考えて動くケアマネさんをたくさん知っています。

介護支援専門員の資格に見る不幸な状況～ケアマネはなぜ評価が低いのか ？

このことに関して「第2回介護支援専門員の資質向上と今後のあり方の関する検討会｣において日本福祉大学の野中構成員は次のように発言しています。（一部僕の解釈を加えています。）

医師や看護師は技術があって制度ができた。つまり医師は技術が先にあるから医師同士の評価ができるが、技術を作ったわけではない厚労省がこの評価をやったら技術は荒廃する。

しかし介護支援専門員は技術の前に制度ができた。だから厚労省は自分たちが作った制度という掌の中にあるケアマネは指導できる。

そうであるがゆえに、その指導とはケアマネジメント技術者ではない、制度設計者の指導なんです。だからどのような優秀なケアマネが全国各地で素晴らしい支援活動を行っていても、国は常に一部のケアマネの支援技術の低さを取り上げて、ケアマネ全体に高評価を与えることはないんでしょう。しかし実際には、介護支援専門員の存在によって、この国の福祉の底辺は確実に引き上げられています。「介護の詩～明日へつなぐ言葉」の156頁の「あなた」とは、介護支援専門員のことを指しています。決して自身の資格を卑下する必要はないのです。
